The Doctor, along her companions (Yaz, Ryan, and Graham), arrive in a dimension where Muppets coexist with superheroes (specifically those that exist in the Marvel Cinematic Universe). Other creations of Jim Henson also exist in this universe, aptly known as the "Earth-M" universe, such as the Moya crew from Farscape, who are allies with the Guardians of the Galaxy.
But the Thirteenth Doctor isn't the only incarnation of the Doctor visiting Earth-M. Over in Wakanda, the Tenth Doctor arrives with Wanda Maximoff and Vision, having rescued them from members of the Black Order that were on the hunt for the Mind Stone, which is embedded within Vision. Miss Piggy, Pepe, Beauregard, and Animal are also in Wakanda, along with S.H.I.E.L.D. Agent Ciciley Livingston and Agent X, a woman who was formerly her son, Sean, before an experiment completely altered his appearance.

Much of the subsequent events that occurred mirror those in Avengers: Infinity War, with the Thirteenth Doctor (along with Ryan, Yaz, Graham, Kermit, his nephew Robin, Fozzie Bear, Scooter, and Gonzo) assisting Iron Man, Doctor Strange, Spider-Man, the Guardians of the Galaxy, and the Moya crew in confronting Thanos on Titan. Prior to this, we learn that Doctor Strange had foreseen two possible victories out of the fourteen million six hundred and five visions. They deduce that the Doctor's presence in Earth-M led to one of the two outcomes.
This much is clear after Iron Man is badly beaten by Thanos, the Thirteenth Doctor and her friends retreat with the Iron Avenger and the Time Stone (given to her by Strange). They are pursued by Thanos, who (whilst hanging onto the Doctor's TARDIS during its flight through the time vortex) is badly injured and loses his gauntlet to the Doctor and her team. He is only able to keep hold of the Space Stone.
Meanwhile, the Tenth Doctor successfully escapes the Black Order invasion over Wakanda with Vision, Wanda, Ciciley, Agent X, and their team of Muppets. Unfortunately, takeoff isn't so smooth, as Ciciley (who distrusts the Doctor) fires on the control console and sends the TARDIS spiraling out of control between the Time Vortex and the Infinite DC. This results in the travelers stranded across the multiverse and the sudden disappearance of Ciciley and Agent X.
In trying to return to Earth-M, the Tenth Doctor and his companions stir up hijinks in a Muppet dimension based on A Christmas Carol, with Wanda accidentally driving Ebenezer Scrooge mad with her powers. They soon receive a distress call from Tony Stark, beckoning this incarnation of the Doctor to come and help his future self, transmitting coordinates to help bring them back to Earth-M.
Meanwhile, the Thirteenth Doctor comes up with a plan to retrieve the Space and Mind Stones, taking her TARDIS to London in 1981, which is when she once visited Earth-M as the Fourth Doctor with his companions, Romana and Adric. The Fourth Doctor is there to consult with Dr. Bunsen Honeydew and Beaker on a multiverse equation of Adric's to help them get out of E-Space. It is Adric who the Thirteenth Doctor reaches over the phone, having him spy on Loki the God of Mischief, who is there in 1981 London by unknown means.

Adric learns that Loki is attending a night at the Dubonnet Club where he intends to steal from Lady Holiday. The Thirteenth Doctor and Tony Stark crash the party, in order to stop Loki and take the Mind and Space Stones from him in the process. Unbeknownst to them, however, Thanos has arrived in 1981 London with his version of the Space Stone, aided by a mysterious hooded figure who promises him control of the multiverse, unless he is successful in his mission with the Infinity Stones.

The plan to steal the Infinity Stones from Loki is met with complications, such as Loki's attempt in enthralling the Doctor and holding Robin (who tailed Stark and the Doctor) hostage. But the plan fails entirely when Thanos arrives at the club, does battle with Iron Man (and wins for the second time), and forces the Doctor to surrender the remaining Stones needed for his victory. The Doctor risks it all for Strange's prophecy of the two outcomes in which Thanos loses. Of course, after Thanos returns to the present, his snap reverberates across time and space and Scooter, Yaz, Ryan, Graham, and Robin are all wiped out of existence.

The devastating loss of her companions has its effect on the Doctor, and it gets worse when she's apprehended by the TVA for "crimes against the multiverse." Her only saving grace is Kara (the thirteenth incarnation of Neas from Infinite DC), who represents the Doctor in trial. The crime that the Doctor is accused is proven to have been committed not by the Thirteenth Doctor but her past tenth incarnation, although the "Charles Dickens" variant of Gonzo (who is the prosecutor) counterargues that all the Doctors are "one in the same." It is only when the Doctor pleads her own case, mentioning Thanos and the Snap, that she is found not guilty - specifically by the Conqueror, the "big boss" of the TVA. Released from captivity, the Doctor returns to Earth-M with Kara and two TVA agents that are revealed to be the lost Agent Ciciley Livingston and Agent X.
